Referrals
At Oral Surgery + Arkansas, we strive to make the referral process as simple, secure, and efficient as possible. Simply follow the steps below, and our team will take it from there.

Referral Instructions
1. Complete the Referral Form
Fill out the referral form with the patient's information and the requested clinical details. You can find the referral form here.
2. Submit Supporting Documentation
Email the completed referral form, the patient's most recent panoramic radiograph (taken within the last 12 months), and any relevant clinical notes via encrypted email to intake@oralsurgeryandwisdomteeth.com.
3. We'll Handle the Rest
Once we receive your referral, our team will contact the patient to coordinate care. If additional information is needed, we'll reach out to your office promptly and keep you informed throughout the treatment process.
All submissions are processed securely in compliance with HIPAA standards.
